Home Care Package Services
Home Care Package (HCP) is a government support program that subsidises costs of at-home care services for eligible senior Australians citizens. We believe that age should not interfere with an individual ability to live an independent life in the comfort of their own home while staying connected with their communities. However, this could be an overwhelming challenge to many senior Australians due to various reasons.
NDIS Services
National Disability Insurance Scheme (NDIS) is a government support scheme that provides fundings for three majors components of disability services : core (daily living), capital (assisted technologies or equipment) and capacity building (skills development) for eligible Australian living with disabilities. Operating under the NDIS, YarraCare provides a wide range of services to assist individuals living with disabilities to maintain their independence.

Care Coordination
Our care coordination service is specifically tailored to your needs. Care coordinator will assist you to build independent skills while receiving ongoing support that strengthen your ability to connect with the community surrounding you.
Care Management
A few of the tasks that your care manager will help you with are booking appointments, making phone calls and basic payments, contacting friends or family members, communicating with other service providers and making basic payments.
In-Home Support
We understand that there is no place like home and we can assist you to stay safely in the comfort of your own home. Our team will provide you with high-quality in-home support services such as personal care, domestic assistance and home maintenance.
Garden Maintenance
Our support worker team can help you with your garden maintenance tasks upon request. Garden maintenance services include helping with veggie gardens, weeding and lawn maintenance.
Respite Care
Respite care programs can be provided by another carer at home, where clients stay in a community or social accommodations. Yarracare also offer flexibility in the duration of your stay so either a day, overnight, weeks or even months.
Transport services
Transport services are available for taking clients to and from appointments. Our personal care staff also have the ability to organise taxis, complimentary shuttle or bus services on client’s behalf.
Meal Preparation
Need assistance with your meals and grocery shopping? Our support worker team could help you with meal preparation and grocery shopping. With our services, you can enjoy your meal without any stress.
Social Connection
We understand the importance of staying socially connected and undertaking social activities that are available to meet your social needs. We can help you to connect with friends, families and your communities.
